Identifying anticholinergic burden in clinical practice

چکیده
Anticholinergic medications are associated with a wide range of adverse effects to which elderly patients particularly susceptible. This article discusses the various ways that anticholinergic burden can be measured, how these used help clinicians identify at risk from anticholinergics, and address risks.

Anticholinergic burden quantified by anticholinergic risk scales and adverse outcomes in older people: a systematic review
BACKGROUND The cumulative effect of taking multiple medicines with anticholinergic properties termed as anticholinergic burden can adversely impact cognition, physical function and increase the risk of mortality. Expert opinion derived risk scales are routinely used in research and clinical practice to quantify anticholinergic burden. These scales rank the anticholinergic activity of medicines ...
متن کاملIncreasing anticholinergic burden and delirium in palliative care inpatients.
BACKGROUND Delirium may complicate the hospital course and adversely impact remaining quality of life for palliative care inpatients. Medications with anticholinergic properties have been linked to delirium within elderly populations via serum anticholinergic assays. AIM The aim of this study is to determine whether increasing anticholinergic burden, as measured using a clinical assessment to...
متن کاملEffect of polypharmacy, potentially inappropriate medications and anticholinergic burden on clinical outcomes: a retrospective cohort study.
BACKGROUND Polypharmacy, potentially inappropriate medications and anticholinergic burden (as assessed by the anticholinergic risk scale) are commonly used as quality indicators of pharmacotherapy in older adults. However, their role in clinical practice is undefined. We sought to investigate longitudinal changes in these indicators and their effects on clinical outcomes. METHODS We used Taiw...
متن کاملCurrent role of anticholinergic drugs in the treatment of asthma: key messages for clinical practice.
Anticholinergic bronchodilators such as tiotropium, a potent long-acting drug, are central to the symptomatic treatment of chronic obstructive pulmonary disease. Its role in asthma treatment has been recently investigated. This review critically evaluates documented evidence of clinical trials and assesses the therapeutic implications of anticholinergic drugs in asthma management. So far, the r...
متن کاملSerum Anticholinergic Activity: A Possible Peripheral Marker of the Anticholinergic Burden in the Central Nervous System in Alzheimer's Disease
We review the utility of serum anticholinergic activity (SAA) as a peripheral marker of anticholinergic activity (AA) in the central nervous system (CAA). We hypothesize that the compensatory mechanisms of the cholinergic system do not contribute to SAA if their system is intact and that if central cholinergic system deteriorates alone in conditions such as Alzheimer's disease or Lewy body deme...
